Why a 'health planner'? Because women often take care of their families'
health and allow little time for their own.
Working with a statewide team of health care professionals and
advisory committee members, we fashioned the Planner into a record-keeper
and health diary with a focus on prevention and self-care. And it's
no larger than an address book!
| Health Planner topics include |
| Selecting and communicating with health care
providers |
| Charting health and wellness |
| Planning for end-of-life-care |
| Tracking your family's health history
and
much more! |
The Planner is not intended to be a comprehensive manual replacing
medical care, but to enhance healthful choices and treatment.
